Whole Lung Lavage under General Anesthesia in A Child with Pulmonary Alveolar Proteinosis (A Case Report)

چکیده
Pulmonary alveolar proteinosis (PAP) is a rare disease in which surfactant accumulates abnormally in the pulmonary alveolar walls and causes respiratory symptoms. The only known effective treatment for PAP is pulmonary lavage. We have reported an 11year-old girl with pulmonary alveolar proteinosis who underwent pulmonary lavage with normal saline under general anesthesia by a new method (using an univent tube for pulmonary blockage, ventilating one lung, and concurrently passing a catheter from out of the tube for lavage). The general condition and vital signs of the patient were normal during lavage and in follow up. Three weeks later, her opposite lung was lavaged too. The results were favorable. Anesthesia for lung lavage in pediatric patient with pulmonary alveolar proteinosis.
BACKGROUND AND OBJECTIVES Pulmonary Alveolar Proteinosis (PAP) is a rare disorder first described in 1958. The Whole-Lung Lavage (WLL) proposed in the 1960s, remains the treatment of choice. Several techniques have been described to perform lung lavage in pediatric patients; however, all have limitation and risks. CASE REPORT Female patient, aged 6 years and 8 months, 25 kg, diagnosed with pu...

Pulmonary alveolar proteinosis is a rare type of interstitial lung disease characterised by diffuse involvement and filling of distal air spaces by the amorphous, periodic-acid-schiff positive lipoproteinaceous material. Whole lung lavage is presently the standard mode of management of such cases. A 33-year-old male presented to our centre with cough and breathlessness on exertion. On evaluatio...
متن کاملSegmental Lung Lavage with Fiberoptic Bronchoscopy in a Patient with Special Presentation of Pulmonary Alveolar Proteinosis
Pulmonary alveolar proteinosis (PAP) is rare. It is characterized by the accumulation of proteinaceous materials in the alveoli. Typical appearance of BAL fluid (BALF) and positive PAS staining of BALF in conjunction with typical clinical and radiographic manifestations may be diagnostic of PAP. The current mainstay of treatment for PAP is whole-lung lavage. Therapy with granulocyte-macrophage ...
متن کاملWhole Lung Lavage of Nine Children with Pulmonary Alveolar Proteinosis: Experience in a Tertiary Lung Center
BACKGROUND Pulmonary alveolar proteinosis (PAP) is a rare disease in children, characterized by intra-alveolar accumulation of large amounts of surfactant proteins, which severely reduce gas exchange. Whole lung lavage (WLL) is the preferred technique for the treatment of severe PAP. CASE PRESENTATION This report presents nine pediatric cases with advanced PAP who underwent WLL under general ...
متن کاملWhole lung lavage for pulmonary alveolar proteinosis after surgery for spontaneous pneumothorax
INTRODUCTION Pulmonary alveolar proteinosis (PAP) is a relatively rare lung disorder, probably under diagnosed, characterized by the accumulation of lipoproteinaceosus material in the lung alveoli. The primary (acquired or idiopathic) form occurs in more than 90% of all cases. Whole lung lavage is considered the golden standard of treatment. In this report, we describe a rare case of pulmonary ...
